Case Manager
This positions functions as a member of the MRHC care team under the supervision of the clinic manager. They will provide support, assistance, linkage, referral, crisis intervention, assessment, outreach and advocacy services to patients referred by the health care team.

Requirements:
· Graduate of an accredited college with a degree in social work, human services and/or related field
· Two years of case management experience in health care setting and/or mental health setting
· Ability to work in a team and to share responsibilities and duties
· Be friendly, empathic and an adept communicator
· Ability to apply sound customer service skills and principles
Job Duties Include:
· Initiates a comprehensive assessment to identify individual needs of the patient.
· Informs patients of available community services and medical health care services and assists with providing them with appropriate information.
· Coordinates clinical issues with health care team and other community professionals to ensure continuity of care.
· Provides education and support to patients and their families by working closely with community agencies to promote and recommend services.

Requirements:
- Medical Office reception or other reception experience preferred.
- Excellent phone skills, able to answer multiple phone lines at once.
- Keyboard and computer skills
- Able to work in a stressful environment with frequent interruptions to job flow, all the while greeting customers with a cheerful and positive attitude

Duties: Answers all incoming phone calls and redirects to proper extension. Schedules and processes the registration function for patients entering our health care organization through the clinic. Secures all information necessary for internal operations. Introduces the patient to the environment, explaining policies, securing signatures, detailing forms, completes billing screens, and answers any questions.
Better Health Improvement Specialist
Full-time (40 hours)
Requirements:
· Bachelor’s level degree in business, healthcare, public health or a related field.
· Consideration will be given to associate degree applicants with considerable experience in healthcare, managed care, public health and project management.
· Strong leadership ability
· Must be able to work some evenings and weekends.
Duties: Communication and operational liaison between the Frontier Medicine Better Health Partnership (FMBHP) and the Mineral Community Hospital. Responsible for becoming an integrated part of the CAH Community, developing an understanding of CAH Community resources and needs, and working to facilitate the creation and implementation of Better Health Improvement Plans, FMBHP Program objectives and individual community goals.
Environmental Services Housekeeper
Part-time (20 hours) Monday-Friday 4:30 pm-8:30 pm
